This paper presents the process and conclusions of the Failure Mode and Effects Analysis (FMEA)
that assesses the critical control points and risks of ultraviolet (UV) disinfection. The FMEA was completed to
support the development of the interactive UV Disinfection Implementation Tool (UVDIT). The UVDIT
is being developed as part of the American Water Works Association Research Foundation (AwwaRF)
Project #2861, Integrating UV Disinfection into Existing Water Treatment Plants. The FMEA results
will be used in conjunction with the information gathered from a utility survey and power quality analysis
to develop the UVDIT. The UVDIT will assist utilities in determining the feasible retrofit locations of
UV disinfection, the risks at the retrofit locations, method for mitigating those risks, and conceptual
retrofit and risk mitigation costs. Includes 3 references, tables, figure.
