Preface

The maturation of vascular surgery into a specialty has allowed for the rapid expansion of the fund of knowledge of diseases of the cardiovascular system. This information explosion has provided a better understanding of the mechanisms of atherosclerosis and other vascular disease processes. Vascular surgeons have played a significant role in elucidating the basic scientific mechanisms underlying these disease processes-a testament to the commitment of practitioners of this specialty.

The field of vascular surgery, although involved in the basic mechanisms of atherogenesis and other vascular diseases, is primarily a clinical specialty. Restoration of unobstructed laminar blood flow remains central to most vascular surgical procedures. Successful outcomes in these procedures demand not only accurate diagnosis and selection of the correct surgical procedure but also flawless execution of the chosen operation in a consistent manner. Surgical technique cannot be learned from a book; hence we have intentionally omitted in-depth technical descriptions from this book. The basic techniques necessary for performing these operations are currently taught in vascular surgery training programs.

Handbook of Vascular Surgery highlights some important aspects of the diagnosis and management of vascular disease. It has not been designed as a textbook of vascular surgery, nor is it meant to be all inclusive. Rather, the reader will gain a basis for understanding a particular problem and some of the alternatives for its management. This book will provide a foundation of information that can, if necessary, stimulate further exploration of the topic in the referenced literature and more detailed vascular texts.

The rapid evolution of this field makes it impossible to cover all of the important topics encountered in vascular surgery. Many of the "cutting edge" advances and less common disease processes have been omitted to allow space for the more common vascular problems. Understandably, any book reflects the bias of its creators. Although a generally "standard" approach has been presented throughout, certain biases are still present. Most contributors to Handbook have been trained in the art and science of vascular surgery by the senior editor. Thus the views presented herein represent the teaching of our vascular surgical group and reflect the testing of these lessons by a long experience in the field.

Handbook is intended to serve a multitude of health care providers well with its straightforward, problem-based approach. Maturing surgeons in training will find the basics with which to approach a specific vascular surgical problem, alerting them to the salient diagnostic points and treatment plans. Established physicians will now have a concise reference so that many of the less obvious points regarding their patient's diagnosis and treatment plans can be recalled. Nursing personnel, interns, and allied health professionals can derive a basic understanding of disease processes and management strategies.

The contributions of all of the authors remain the heart of this book. Their intellect and diligence are reflected in the pages of Handbook of Vascular Surgery. A sincere thanks is extended to Michael Torres, whose photographic contributions have been invaluable. Another important person in producing this book is Maxine D'Amelio. Her dedication, insight, and resourcefulness in editing and preparing manuscripts have earned our deepest appreciation.
