Purpose: This worksheet shall be completed by the test requestor. It is intended to specify test instructions to the lab facility which will perform the test.

• Sections as shown in the template shall stay in the same order. Sections shall not be deleted.

• The test requester shall modify areas marked (TBD) or shown in Bold Blue font, or as needed to customize the Test Plan for their application.

• All test requester inputs shall appear in Bold Blue font.

• To indicate the selection of a check box, select the box entirely using the mouse cursor and then replace it with an “X” typed in by the user. (Note: Clicking on the box will not “select” it.)
