This standard specifies a national safety related event reporting management process for all of Network Rail’s M&EE disciplines and covers all maintenance, renewal and new works activities including Network Rail’s maintenance and operation of trains and rail vehicles. It is applicable to:
• Route Businesses England & Wales
• Route Business Scotland
• Safety, Technical & Engineering
• Telecoms Asset Management, Digital Railway
• Infrastructure Projects
• National Supply Chain, Technical Services
• Works Delivery
• Contractors employed by any of the above
Purpose
This standard provides a common safety related event (SRE) reporting process for M&EE comprising Power Distribution HV/LV, Contact Systems AC/DC, Traction and Rolling Stock (T&RS) and Plant assets. It will provide:
• A key assurance process for Network Rail’s Board.
• High level data, published in the Safety, Health & Environment Performance report (SHEP) that is publicly available on line.
• An auditable process to meet the requirements of The Management of Health and Safety at Work Regulations 1999 as amended and The Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR).
• Alignment with the Safety Management Intelligence System (SMIS+), which enables the rail industry to make evidence-based decisions supported by high quality data.
• Assurance that the root cause of SREs has been identified, existing risk controls are suitable and sufficient, and where this is not the case, that appropriate remedial action is taken.
