This standard applies to all Network Rail led local and formal investigations.

Purpose

To define the process to be used within Network Rail for producing summarised reports of investigations into safety events.

Investigation reports should be comprehensive in order to justify their conclusions and recommendations. However, this level of detail may not be appropriate for all audiences and may, in some cases, inhibit understanding of the key issues. In addition, a single investigation report may not identify important trends in safety events.

The aim of producing a summarised investigation report is to provide a report that:

a) targets a specific audience(s);

b) contains an adequate, but not excessive, level of detail;

c) presents the key issues in a way that can easily be comprehended;

d) identifies any important trends.
