This procedure applies to all Network Rail Managed Stations.
A full range of various incidents and events that may warrant the need to complete a SMIS are listed in Form FS-13A. A SMIS incident is anything that affects the safe running of the operational railway or the running of Network Rail properties, including Managed Stations. The general events where this information may be required are as follows:
• environmental events;
• property damage;
• incidents and near misses;
• train dispatch incidents;
• buffer incidents (resulting in injury or significant damage);
• instances of trolleys, tow-tractors, buggies etc. falling from the platform onto the line, or striking trains while manoeuvring on platforms;
• instances of trespass and vandalism;
• occasions when the station is evacuated due to other emergencies;
• overcrowding (refer to NR/L3/OCS/044-MS-32 for crowd control levels of disruption for information);
• workforce accidents and assaults;
• public accidents and assaults;
• irregular working.
PURPOSE
To provide a consistent process to the reporting of events on all Network Rail Managed Stations that will enable the capture of timely and accurate information. This will help Network Rail in assessing safety risks, monitoring safety performance and supporting the investigation process. Click here for Accident Reporting Forms.
