This procedure applies to Route Operations Control and the following:
Signals.
Stop boards or Indicators.
Limit of Shunt Signals or Indicators.
Position Light and Shunting signals where the normal aspect means ‘stop’ and the movement is in the direction for which the signal can be cleared.
Possession Limit Boards.
Stop indications given by a hand signaller.
ERTMS End of Authority (EoA) usually indicated by an ERTMS block marker that are passed at danger without authority, on Network Rail Managed infrastructure.
PURPOSE
This National Operating Procedure sets out Network Rail’s incident response to alleged Signal Passed At Danger (SPAD) events. It defines the process for:
a) producing SPAD Alerts;
b) undertaking SPAD Risk Ranking; and
c) managing the production of Multi-SPAD summary briefing information.
It contains details of Signals Passed at Red (SPAR) events and how Network Rail responds to such events.
The procedure makes sure that each SPAD and SPAR is managed in a way which prevents increased or new risks, as well as making sure that they are investigated thoroughly to establish the root cause and prevent reoccurrence.
