This procedure applies to all Permanent Way asset failures as described in TEF3064 which are identified and reported through the Control Log. This procedure does not report defects or potential hazards which are identified and managed by routine inspection thus avoiding the requirement of any form of traffic protection.

The Track Maintenance Engineers shall review and assess hazards identified in the Control Log. Analysis of the hazards reported is undertaken at levels from Track Maintenance Engineers through to the Director of Track Engineering team to help identify any emerging issues.

Failures with a Hazard Rating greater than or equal to 50 are reported individually with additional supporting information in Part 2 of TEF3064. Preliminary versions of these forms will also be sent, within one working day, to the Director, Maintenance (as well as the Route Infrastructure Maintenance Director, the Infrastructure Maintenance Engineer, Director of Track Engineering and the Principal Maintenance Support Engineer [Track]

Failures with a Hazard Rating greater than or equal to 50 are elevated to the Chief Engineer and the Board as part of the Safety, Environment & Assurance report (SEAR).

Hazards rating greater than or equal to 50 will be reviewed, along with hazard trend analysis, at the Director of Track Engineering's team meeting.

Purpose

This document describes a numerical system for Hazard Ranking of Permanent Way asset failures. The system is based on the principles of the Network Rail Standard RT/E/S/10047: Management of Safety Related Reports for Signalling and Operational Telecom (S&T), which ranks failures and incidents on a scale of 0 to 228 based on impact to operational safety.

The Hazard Ranking System for Permanent Way asset failures has been developed in response to a requirement from the Network Rail Board to have a Hazard Ranking System in place for all asset types. This is to achieve uniformity in the quantitative reporting of hazards across infrastructure types and to aid Company awareness of failures having a Hazard Rating greater than or equal to 50.
