Introduction
In recent months a number of TPWS brake application faults have been noted in FMS where the train has been stopped on it's approach to the associated signal displaying a proceed aspect. They all occurred following the removal and re-installation of TPWS loops for PWay work.
Investigations revealed that the Arming and Trigger loop grid assembly plug coupled cables had been incorrectly crossed when re-installing the loops.
These TPWS brake application faults are not in themselves Wrong Side Failures.
What is of concern is the potential failure mode for a train travelling in the opposite direction on the bi-directional line. If it were to pass the signal at danger, then no TPWS intervention would occur, resulting in a Wrong Side Failure.
The SMTH testing conducted failed to reveal this potential Wrong Side Failure.
