Purpose and scope of the recommendation

Within a programme, the information linked to problems with the products, processes and associated resources, which occur throughout the lifecycle of these products (confirmed problems) must be methodically analysed with a view to eradicating the causes of these incidents and treat their effects.

The guiding principle is to define a system for collection and processing of these data, within the framework of the customer/supplier relations, at whatever level.

This recommendation is designed to constitute a basis, within a given programme, for defining, setting up and implementing a failure reporting analysis and corrective actions system (FRACAS).

This document is therefore intended for supplier programme or project managers, more particularly for those responsible for defining and implementing this logic within their management teams (quality assurance, dependability and safety, etc.).

This recommendation can also constitute a basis for choosing the extent to which this FRACAS is to be formalised, depending on the specificity of the programme.

This recommendation is usable for any programme involving various actors at various levels, in particular in the case of large programmes (aerospace, weapons, etc). It can also be used to manage programmes launched with no outside contract.

The FRACAS applies to any incident occurring during a programme or during the lifecycle of a product developed and built under this programme and in particular during:

— the design of the product;

— construction;

— procurements necessary for manufacturing;

— testing;

— delivery;

— storage;

— operation;

— maintenance;

— product retirement from service and disposal.

It also concerns the components of these products and the associated means and processes implemented.

It is up to the programme managers to decide on what is to be adopted or adapted in order to optimise attainment of their targets.

The FRACAS requirements adopted for a given programme must be described in the management plan produced in response to the programme management specification.
