This booklet is a reprint of the previous Pt.3 Ch.2 (January 2003 version). Apart from the changes described under “Main changes” and the inclusion of amendments and corrections, published in the July 2006 edition of Pt.0 Ch.1 Sec.3, no other changes have been made.

This chapter is valid until superseded by a revised chapter. Supplements will not be issued except for an updated list of minor amendments and corrections presented in Pt.0 Ch.1 Sec.3. Pt.0 Ch.1 is normally revised in January and July each year.
