This Special Instruction Notice is applicable to Regional Directors of Engineering and Asset Management and their teams and applies to all Polecat 570 CCTV winch assemblies in all routes.

The SIN includes the requirements to replace the Polecat 570 CCTV winches numbered W001 to W451 and also the requirement to inspect all other winches in line with the guidance given in Appendix E.

The scope includes a visual inspection of the CCTV System and a review of the associated Work Activity Risk Assessments and procedures for safe isolation and locking off when maintenance activities are undertaken.

The scope includes the provision of a priority driven action plan to address deficiencies identified.

Purpose

This Special Instruction Notice has been produced in response to a recent incident at Rigton CCTV Tower, North & East Route, where a member of staff received minor injuries  whilst undertaking machinery maintenance activities.

It’s a requirement under health and safety legislation that all work equipment is safe to use and that risks to those who use or maintain the equipment have been identified and appropriate control measures implemented.

A systematic review of all Network Rail owned CCTV systems across all Regions and Routes is required to identify all CCTV systems that present a similar hazard to the workforce.

In addition, an inspection of the winch cable and CCTV mounting platform is required.

There may be modifications required to the system due to changes in the type and mass of cameras that have been fitted to the carriage since it was originally installed.
